This is a New Meyer Drive Pro Mount #18502 for a 2005-2015 Toyota Tacoma 4x4. This mount was replaced by the #18533 mount to fit the 2016 as well, but this is a chance to get a cheaper one.
It comes with Left Hand Side Member Weld #19907, Right Hand Side Member Weld #19908 and Hardware Bag #08149.
The Clevis (shown below) is not included, but if you do need that part it is part #19943.
