This is a Meyer hardware bag 08796. It is a set of 4 Meyer Mount Washer Plates (part # 11139.)

These are used on Meyer EZ Classic mounts (17103, 17104, 17116, 17117),  EZ Plus/MDII Mount 17123, and Meyer Xpress mount 17145.

They are 7/16" washers. This set goes by hardware bag # 08796 and includes all four you will need for each mount. To buy a single one of these (11139) from Meyer is $26 alone.
